Cardiac surgery is performed on hundreds of thousands of patients a year, and can have an important beneficial impact on the outcomes of patients with coronary and valvular heart diseases. Despite the favorable recovery of most patients, some will have their post-operative period interrupted by the development of atrial fibrillation, with a host of potential complications including stroke. High risk subgroups may develop atrial fibrillation in more than half of cases, and often despite aggressive prophylactic measures. Treatment of atrial fibrillation and its aftermath can also add days to the hospital stay of the cardiac surgical patient. In an era of aggressive cost cutting and optimization of utilization of health care resources, the financial impact of this arrhythmic complication may be enormous. Experimental studies have led to a greater understanding of the mechanism of atrial fibrillation and potential precipitating factors in the cardiac surgical patient. Prophylactic efforts with beta-blockers, antiarrhythmic drugs and atrial pacing are being used, or are being investigated in clinical trials. New methods of achieving prompt cardioversion with minimal disruption of patient care, and prevention of the thromboembolic complications of atrial fibrillation, are also important therapeutic initiatives. This text is designed to aid health care professionals in the treatment of their patients in the recovery period after cardiac surgery, and to instigate additional research efforts to limit the occurrence of, and the complications following, this tenacious postoperative arrhythmia.

In the midst of human-induced global climate change, powerful industrialized nations and rapidly industrializing nations are still heavily dependent on fossil fuels. Even if we arrive at a Hubbert’s peak for oil extraction in the 21st century, the availability of technologically recoverable coal and natural gas will mean that fossil fuels continue to be burned for many years to come, and our civilization will have to deal with the consequences far into the future. Climate change will not discriminate between rich and poor nations, and yet the UN-driven process of negotiating a global climate governance regime has hit serious roadblocks. This book takes a trans-disciplinary perspective to identify the causes of failure in developing an international climate policy regime and lays out a roadmap for developing a post-Kyoto (post-2012) climate governance regime in the light of lessons learned from the Kyoto phase. Three critical policy analytical lenses are used to evaluate the inherent complexity of designing post-Kyoto climate policy: the politics of scale; the politics of ideology; and the politics of knowledge. The politics of scale lens focuses on the theme of temporal and spatial discounting observed in human societies and how it impacts the allocation of environmental commons and natural resources across space and time. The politics of ideology lens focuses on the themes of risk and uncertainty perception in complex, pluralistic human societies. The politics of knowledge lens focuses on the themes of knowledge and power dynamics in terms of governance and policy designs, such as marketization of climate governance observed in the Kyoto institutional regime.

The cardiovascular benefits of dietary omega-3 polyunsaturated fatty acids (n-3 PUFA) have been actively investigated for nearly 40 years. Beginning with the pioneering studies of Bang and Dyerberg, epidemiological data provide strong evidence for an inverse relationship between fatty fish consumption and cardiac mortality. In contrast to these observational studies, interventional studies using n-3 PUFAs for the secondary prevention of adverse cardiovascular events in patients with heart disease have yielded conflicting results; some studies have reported reduced sudden cardiac death or mortality, while other more recent studies have reported either no effect or an increase in adverse cardiac events. Nevertheless, the American Heart Association and the American College of Cardiology continue to recommend fish oils for the secondary prevention of coronary artery disease. Based in part upon these recommendations, consumer demand for n-3 PUFA products (both nutritional supplements and foods enriched with these lipids) has exploded. In the United States alone, it has been estimated that in 2004, 5-10% of the adult U.S. population were taking a fish oil supplement, with annual sales growth of 40%. In fact, the sales of these products are projected to exceed 7 billion dollars by the end of 2011 (www.marketresearch.com, product reports). Despite the extensive marketing of fish oil products, a scientific consensus on the effects of n-3 PUFA on cardiac rhythm has yet to be reached. It is the purpose of this Research Topic to stimulate a discussion on the putative benefits of n-3 PUFAs on cardiac rhythm. Authors are invited to submit clinical, translational, or experimental research articles, reviews, and hypotheses that address the actions of n-3 PUFA (positive, negative, or neutral) on cardiac rhythm and cardiac electrophysiology. Studies that evaluate the effects of n-3 PUFA on myocyte electrical properties, atrial fibrillation, ventricular fibrillation, and heart rate variability are particularly welcome.

The calcium-calmodulin dependent protein kinases (CaMKs) are a broadly expressed family of calcium-sensitive intracellular kinases, which are responsible for transducing cytosolic calcium signals into phosphorylation-based regulation of proteins and physiological functions. As the multifunctional member of the family, CaMKII has become the most prominent for its roles in the central nervous system and heart, where it controls a diverse range of calcium-dependent processes; from learning and memory at the neuronal synapse, to cellular growth and death in the myocardium. In the heart, CaMKII directly regulates many of the most important ion channels and calcium handling proteins, and controls the expression of an ever-increasing number of transcripts and their downstream products. Functionally, these actions are thought to orchestrate many of the electrophysiologic and contractile adaptations to common cardiac stressors, such as rapid pacing, chronic adrenergic stimulation, and oxidative challenge. In the context of disease, CaMKII has been shown to contribute to a remarkably wide variety of cardiac pathologies, of which heart failure (HF) is the most conspicuous. Hyperactivity of CaMKII is an established contributor to pathological cardiac remodeling, and is widely thought to directly promote arrhythmia and contractile dysfunction during HF. Moreover, several non-failing arrhythmia-susceptible phenotypes, which result from specific genetic channelopathies, functionally mimic constitutive channel phosphorylation by CaMKII. Because CaMKII contributes to both the acute and chronic manifestations of major cardiac diseases, but may be only minimally required for homeostasis in the absence of chronic stress, it has come to be one of the most promising therapeutic drug targets in cardiac biology. Thus, development of more specific and deliverable small molecule antagonists remains a key priority for the field. Here we provide a selection of articles to summarize the state of our knowledge regarding CaMKII in cardiac health and disease, with a particular view to highlighting recent developments in CaMKII activation, and new targets in CaMKII-mediated control of myocyte physiology.
