These congenital deformities, funnel or keel chest deformities, as well as Poland syndromes, affect a small group of patients who suffer from aesthetic rather than functional impairment. The wide variety of diagnoses and recommended therapies, mostly surgical procedures, can be very confusing for patients, their parents, physicians and surgeons. In recent years several techniques have been refined and developed to deal with these complex problems, and surgeons with different specialties have been operating on these deformities, mostly as lonely experts in their respective fields. This book should not be seen as an operation atlas but more as a compact survey of a small group of medical conditions and the need for flexible options for an individual therapeutic approach, based on the combined experience of different international specialists. Thus the book is designed for obstetricians, pediatricians, physicians and surgical specialists alike.

This book discusses predominantly pectus excavatum and pectus carinatum, its variation and the treatment modalities available, including minimally invasive surgery. This will help to increase awareness of the available treatment options to doctors and healthcare workers but will also help trainee cardiac and thoracic surgeons. It is also an essential reference book for those already practicing. Chest wall deformities are a common occurrence, the commonest being pectus excavatum and pectus carinatum. The treatment for these is provided by very few centers and there is a decline in expertise especially in the UK due to rationing. When patients approach their GP they are mostly turned away and asked to live with it due to a lack of awareness and treatment modalities. The Editors have extensive experience in this field and will invite authors from other specialties, i.e. chest physicians, cardiologist, pediatric surgeons, anesthesiologists, pain management team, plastic surgeons and prosthetic departments, to contribute their views and solutions to the book.

This book was conceived and written to provide a contemporary view of critical urban transport issues, policies, and initiatives in twelve countries with emerging economies, each at somewhat different stages of development. With dedicated chapters on Brazil, China, Colombia, India, Indonesia, Iran, Mexico, Nigeria, Russia, South Africa, Turkey, and Vietnam, the book contains detailed, comparable information about the current urban transport situation in the major cities in these countries. Written by specialists in the field, the book draws on a wide range of information sources to provide up-to-date accounts of each of these countries. By assembling this information in one volume, it provides a valuable source for academics as well as policy-makers with an interest in the current and emerging urban transport needs of a large portion of the world’s population.

Children present a variety of deformities of the thoracic cage which can be classified into two main groups: congenital (abnormal development) and acquired. The purpose of this paper is to summarize clinical observations and the successful surgical treatment of congenital funnel chest in children. Funnel chest consists in the depression of the central portion of the thoracic cage and of the epigastric portion of the abdomen. This depression is due to the retraction of the sternum or to be more exact of the xiphoid. The deformity, as a rule, involves the entire costal cartilages up to the mamillary line. The deformity may be symmetrical or asymmetrical. Different degrees of depression are to be observed depending on the amount of the deformity and the age of the child. (Author).

Poland s Anomaly Natural History and Long Term Results of Chest Wall Reconstruction in 33 Patients

Poland's anomaly is an uncommon congenital aberration of the chest wall characterized by absence of the pectoralis major muscle and other nearby musculoskeletal components. In this series, a wide spectrum of thoracic deformities was associated with the Poland anomaly, ranging from segmental agenesis of the ribs, sternum, and nearby muscles, to simple aplasia of the pectoralis major muscle. Although little disability was associated with the syndrome, the patients primarily sought operative correction due to the asymmetry and their perception of adverse cosmesis. Over a 10-year period, 53 operations were performed on 27 individuals with the goal of correcting the abnormal contour of the chest wall. The most successful reconstructions involved the use of the latissimus dorsi muscle, which was detached and transferred to the anterior chest wall while preserving the neurovascular pedicle. In women, this was accomplished by insertion of a mammary prothesis. Reconstruction of the so-called 'herniation' of the lung with the rib grafts or alloplastic materials was found to be unnecessary, and the use of custom-made chest wall prosthesis is not recommended, since four of five of these devices had to be removed due to migration, erosion of local tissues, and adverse cosmesis. The Aim of "Current Concepts of General Thoracic Surgery" is to provide a brief overview of several topics in this field. It includes a collection of contributions from many outstanding Authors who provide their knowledge and experience from many countries around the world. We apologize for the chapters reviewed that have were not chosen for publication in this book; however, according to the single centres experience, the final result offers thorough and precious information on the several topics evaluated by the Authors. The wide range of subjects discussed goes from CT assessment of solitary pulmonary and metastatic nodules to prospective studies of drug delivery in thoracic surgery including surgical risk prediction, stress reaction, robotic pulmonary and cardiac procedures, vascular and thoracic reconstruction techniques, thoracic trauma and mediastinal fistula. I believe that this book represents an enhancement in the knowledge and in the involvement of individuals dedicated to these areas of study. It is my duty and pleasure to thank colleagues who helped me in the interesting and stimulating review process; Dr. Stefano Pasquino for cardiac surgery and Professor Francesco Puma for his many worthwhile suggestions.
