The motivation for this book was the recognition by two of us (RL and TL) that, despite our training as cardiologists, we had a limited understand ing of many aspects of the surgical management of our patients. Conversa tions with other cardiologists at our own institution and at other medical centers around the country convinced us that many of our colleagues shared our uncertainty about the details of surgical procedures and the factors that determine decisions in the perioperative period. As surgical techniques continue to evolve, this knowledge gap can become only more severe. We therefore compiled information about cardiac surgery that might be useful for cardiologists and other nonsurgeons. We asked cardiologists what questions they would like to ask their surgical colleagues and provided these questions to the authors of the chapters. Thus, the goal of these chapters is to help nonsurgeons answer their own questions about common cardiac procedures, and to help prepare these physicians to address the questions raised by their patients. In addition, we hope that insight into these issues will improve communication between surgeons and their col leagues-and, ultimately, lead to better patient care. The #1 New York Times bestselling author of My Sister's Keeper examines the fault lines of a troubled marriage in this “unfailingly intelligent…undeniably literary psychological drama”(Booklist). To the outside world, they seem to have it all. Cassie Barrett, a renowned anthropologist, and Alex Rivers, one of Hollywood's hottest actors, met on the set of a motion picture in Africa. They shared childhood tales, toasted the future, and declared their love in a fairy-tale wedding. But when they return to California, something alters the picture of their perfect marriage. A frightening pattern is taking shape—a cycle of hurt, denial, and promises, thinly veiled by glamour. Torn between fear and something that resembles love, Cassie wrestles with questions she never dreamed she would face: How can she leave? Then again, how can she stay?

In the early days of cardiac surgery, but also in the following four decades, median sternotomy and cardiopulmonary bypass were nearly always included in cardiac surgical procedures. Less invasive surgical techniques were developed with the aim of reducing perioperative trauma without compromising the surgical result, and things became more complex for cardiac surgeons. They now often had to consider the surgical access of choice and whether cardiopulmonary bypass should be used or not. Since the mid 1990s, not only have several novel minimally invasive surgical techniques been presented, but also further refinements have been recommended from time to time. This work comes to fill a gap in the field of coronary artery bypass grafting and conduit harvesting in cardiac surgery, by gathering the mature version of such new, less invasive techniques combining safety, effectiveness, simplicity, sometimes even reducing procedural costs and that always for the patient’s and for the surgeon’s sake.

Malignancies are frequent complications in organ transplantation, mainly as the result of infection with certain viruses and of long-term immunosuppression. The epidemiology confirms that the increased incidence concerns certain cancers, especially HIV-related skin cancers and EBV-related lymphoproliferative malignancies. This book covers all currently available information on this important topic of the relationships between transplantation and malignancies: preexisting cancers, posttransplant cancers, their etiology and pathophysiology, their prevention and treatment. A significant part of the volume is devoted to prophylaxis, early detection and modern forms of therapy in posttransplant lymphomas. As a conclusion of all these new data, the theory of immunosurveillance deserves to be significantly modified.

The Second Vatican Council declared the restoration of unity among Christians to be one of its principal concerns. Division among the Churches scandalises the world and damages that most holy cause, the preaching of The Gospel to every creature. A key figure to have worked tirelessly at these aims is Cardinal Walter Kasper, one of the most experienced, authoritative, and respected members of the Vatican hierarchy. Here Cardinal Kasper summarises the results of the Catholic Church's official dialogues with Lutheran, Reformed, Anglican and Methodist Churches over forty years, sets out a blueprint for 'consensus and convergence', and sets out a map for the way forward for ecumenism. From such a source, this document is therefore of crucial importance. The book focuses on the major issues in ecumenical dialogue - Jesus Christ, The Holy Trinity, Justification, The Church and The Sacraments of Baptism and Eucharist. Cardinal Kasper explains the key issues at stake and offers signposts for the way forward so that the wish of Jesus may be fulfilled: That They May All Be One.

Important and provocative, The Undead examines why even with the tools of advanced technology, what we think of as life and death, consciousness and nonconsciousness, is not exactly clear and how this problem has been further complicated by the business of organ harvesting. Dick Teresi, a science writer with a dark sense of humor, manages to make this story entertaining, informative, and accessible as he shows how death determination has become more complicated than ever. Teresi introduces us to brain-death experts, hospice workers, undertakers, coma specialists and those who have recovered from coma, organ transplant surgeons and organ procurers, anesthesiologists who study pain in legally dead patients, doctors who have saved living patients from organ harvests, nurses who care for beating-heart cadavers, ICU doctors who feel subtly pressured to declare patients dead rather than save them, and many others. Much of what they have to say is shocking. Teresi also provides a brief history of how death has been determined from the times of the ancient Egyptians and the Incas through the twenty-first century. And he draws on the writings and theories of celebrated scientists, doctors, and researchers—Jacques-Bénigne Winslow, Sherwin Nuland, Harvey Cushing, and Lynn Margulis, among others—to reveal how theories about dying and death have changed. With The Undead, Teresi makes us think twice about how the medical community decides when someone is dead.

From the author of Mayflower, Valiant Ambition, and In the Hurricane's Eye--the riveting bestseller tells the story of the true events that inspired Melville's Moby-Dick. Winner of the National Book Award, Nathaniel Philbrick's book is a fantastic saga of survival and adventure, steeped in the lore of whaling, with deep resonance in American literature and history. In 1820, the whaleship Essex was rammed and sunk by an angry sperm whale, leaving the desperate crew to drift for more than ninety days in three tiny boats. Nathaniel Philbrick uses little-known documents and vivid details about the Nantucket whaling tradition to reveal the chilling facts of this infamous maritime disaster. In the Heart of the Sea, recently adapted into a major feature film starring Chris Hemsworth, is a book for the ages.
